Hollis Parents & Guardians:

Each year parents, guardians, and students are asked to review some standard school practices and policies. Usually, these policies are sent home in paper form during the first few weeks of school. Instead of tracking paper forms, we are trying to complete this task electronically this year. By completing the form linked below, you will acknowledge your agreement to the school’s Internet Acceptable Use Policy, receipt of School Handbook, Photo & Video Release, and Field Trip Permission Form.

One form should be completed for each child enrolled at Hollis Grade School.
